What You Need to Bring for Your Fingerprinting Appointment in Orange County

 At Orange County Live Scan, we make the fingerprinting process as smooth and fast as possible. To ensure your Live Scan appointment is successful, please review the important requirements listed below. Being fully prepared helps avoid delays and ensures your fingerprints are submitted to the appropriate agencies without issue.

 

Live Scan Request Form (BCIA 8016)


This is the most important document you must bring.


  • This form is provided to you by the requesting agency, such as your employer, licensing board, school, or government agency.
     
  • It contains vital routing information such as:

    • ORI code
       
    • Agency authorized to receive results
       
    • Applicant type (e.g., employment, license, volunteer)
       
    • Job title or reason for request
       
  • It must be filled out clearly and completely prior to fingerprinting.

Valid Government-Issued Photo Identification


You must present a valid and unexpired government-issued photo ID to verify your identity before fingerprinting. Acceptable forms of ID include:


  • California Driver’s License
     
  • California Identification Card
     
  • U.S. Passport or U.S. Passport Card
     
  • Military ID (Active, Reserve, Retired)
